- PLEASE NOTE: If you are requesting the use of one of our fields or facilites and you are a league, a CORI form and a copy of a current drivers license must be submitted for each and every person who is over the age of 18 and is involved in the league. This includes but is not limited to, coaches, team parents, volunteers, concession stand workers. The CORI forms may be dropped of at the Seekonk Police Station, Attn: Dennis LeClerc. A copy of your leagues insurance certificate must be submitted with your request form.
- Field / Facility requests are NOT first come first serve. Town sponsored leagues have the first rights to the fields/facilities, ouside of the school department.
